Family Ministry Registration Form

Thank you for worshipping with us! If this is your first time at Carolina Forest Community Church, or if you have updates to your family's information, please fill out the form below. We can add additional information about your household members, such as age/grade of children and any medical notes or special instructions, when you check in at the kiosk before service.

Listed below are the persons, other than myself or my spouse, which I give permission for Carolina Forest Community Church to allow to pick up my child at the end of the service. These persons will still need to have the numbered sticker tag for the child in order to pick up the child. If the leader does not recognize the person you are authorizing, the person will be asked to show a photo ID. Children will only be released to older siblings if they are listed below and have the number tag. Please list the person's full name, as well as relationship to the child. If you do not wish any other persons to pick up your child(ren) from class, please write "NONE".

Occasionally, Carolina Forest Community Church will take photos or videos of participants in our Sunday morning programming, weekly groups, or other events. We sometimes use these photos/videos in such ways including, but not limited to: displayed in our church buildings, on our website or social media accounts, or on printed materials. Below is the legal agreement for us to use any photos or videos of you or your children at any of our events.

"I hereby consent that my image, likeness and/or voice as depicted or recorded in video productions, audio recordings, television programming, photographs and/or electronic images, and/or written or verbal quotes/comments (collectively, the “Contribution”) may be used by Carolina Forest Community Church in any media product, production, and church website in its sole discretion (collectively, the “Production”).

I acknowledge that Carolina Forest Community Church will own sole copyright to the Production, and I, on behalf of myself and my heirs, successors, and assigns, release Carolina Forest Community Church, its officers, directors, employees, agents, and affiliates from any claim for royalties, copyright, or other infringement, defamation, invasion of privacy or of right of publicity, or other claim arising from any use of the Contribution in any medium, now known or hereinafter created, by Carolina Forest Community Church or its licensees or assignees, including and without limitation, marketing uses which endorse Carolina Forest Community Church or its programs or products.

I acknowledge that Carolina Forest Community Church may edit or revise any part of the Production that contains the Contribution without the need for additional approval."

Sensory Room

Children are grouped in their classes by their age/grade level for fun, creative, and age-appropriate Bible teaching! For children who may benefit from a quieter, less active environment, with more individualized attention, we also offer a Sensory Room during our 10:30 service. If your child would benefit from this type of environment, please check the box below and we will follow up with you before you arrive.
